Public Records Request Form
Estacada Fire District is committed to transparency and to providing the public with access to District records in accordance with Oregon Public Records Law (ORS 192.311 to 192.478). To request records, please complete the form below and describe the records you are seeking as specifically as possible, including dates, addresses, incident numbers, and the names of involved parties where applicable. Clear, specific requests help us respond more quickly.
The District will acknowledge your request within five business days and will respond as soon as practicable, generally within ten business days of that acknowledgment, as provided in ORS 192.324 and ORS 192.329. Complex or large requests may require additional time, in which case we will provide an estimated completion date.
Fees may apply. As permitted by ORS 192.324(4), the District may charge fees reasonably calculated to recover the actual cost of making records available, including staff time to locate, review, redact, and copy records. If the estimated cost exceeds $25, we will provide a written estimate and will not proceed until you confirm that you wish to continue. Payment may be required before records are released.
Some records, or portions of records, are exempt from disclosure under state or federal law, including ORS 192.345 and ORS 192.355. Records containing protected health information, such as patient care reports from emergency medical responses, are confidential under the Health Insurance Portability and Accountability Act (HIPAA) and ORS 192.553 to 192.581 and will be released only to the patient, the patient's authorized representative, or as otherwise permitted by law. Proof of identity or a signed authorization may be required. Where a record contains both exempt and non-exempt information, the District will redact the exempt material and release the remainder.
The District is not required to create new records, compile data, or answer questions in response to a request. Submitting this form does not guarantee that the requested records exist or will be released. If your request is denied in whole or in part, you will be notified in writing of the reason and of your right to seek review by the Clackamas County District Attorney under ORS 192.415.
